Appointments
If you would like to make an appointment, please contact our office by phone or email. Our office facilitates communications and emergency calls and appointments are available and welcome, especially for new patients seeking help.
Appointments are available on/during Mon./Wed. 10am-12:30pm & 3-7 pm, Tues. 3-7pm and Sat.10:00 am-1:30pm. Friday 10 am - 3pm. Tues. morning and Thursdays the office is closed. Please make appointments during the hours posted. Emergencies please leave message at office.
Financial
Only you, the patient, can put the value on your health and well-being. Any "out of pocket" cost to you is "truly" an investment in yourself. Please note, the majority of health insurance today is based on "sick care" not health at all. The office accepts "most" insurance carriers as an "Out of Network" provider. We are enrolled with "Medicare".
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. Knowing your coverage for Chiropractic care before coming in would be an advantage. **The patient is responsibie for all deductibles, co-pays and any changes to their policy. The office has no responsiblity of those charges or any changes in policy.
We will do everything we can to help you afford the treatment you need and would benefit from. Other forms of payment accepted by the office are: Check, Cash, Visa and Mastercard credit/debit cards only.
For all personal checks "returned" all patients' are responsibile for any bank fees and a $20 office fee that apply. Any over due balances of 30 days are subject to a $30 late fee. Any deliquent accounts will be turned into a collection agency for recovery.
All payments rendered at time of service, unless different arrangements have been made with the office.
We ask that all HORIZON BLUE CROSS BLUE SHIELD checks with their EOBs that are sent directly to the patient for office services must be endorsed and brought into the office for accounting. Horizon BCBS's policy is to send checks directly to you the member not to the provider.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
