Only you, the patient, can put the value on your health and well-being. Any "out of pocket" cost to you is "truly" an investment in yourself. Please note, the majority of health insurance today is based on "sick care" not health at all. Please note the "third party payer" (Chiropractic Management,Co.) it is a waste of your time and our effort to give you the proper and quality Chiropractic care you deserve. A third party payer "mission statement" is to restrict and deny Chiropractic care, basically, saving money for the insurance company who hire their services. The office accepts "most" insurance carriers as an "Out of Network" provider and we are enrolled with "Medicare". If you have no insurance coverage this office can make arrangements to make your Chiropractic health care affordable.

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. Knowing your coverage for Chiropractic care before coming in would be an advantage. **The patient is responsibie for all deductibles, co-pays and any changes to their policy. The office has no responsiblity of those charges or any changes in policy.  Please notify us to any change in your policy.

We will do everything we can to help you afford the treatment you need and would benefit from. Other forms of payment accepted by the office are: Cash, Checks, Visa, American Express, Discover and Mastercard credit/debit cards only.

For all personal checks "returned" all patients' are responsibile for any bank fees and a $20 office fee that apply. Any over due balances of 30 days are subject to a 1.5% finance charge. Any deliquent accounts will be turned into a collection agency for recovery.

All payments rendered at time of service, unless different arrangements have been made with the office.

We ask that all HORIZON BLUE CROSS BLUE SHIELD checks with their EOBs that are sent directly to the patient for office services must be endorsed and brought into the office for accounting. Horizon BCBS's  out of network policy is to send checks directly to you the member not to the provider.

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. The cancellation policy is as follows: 3 appointments that are made and the office does not receive calls to reschedule or cancel, the patient is discharged from care.
